Why does your shoulder hurt on days you did nothing?

Shoulder pain on an easy day usually comes from repetition, not effort. Hours with the arms forward at a keyboard, on a steering wheel or holding a phone keep some shoulder muscles switched on and others idle, and the joint stops moving the way it was built to.

Why does my shoulder ache when I did nothing to it?

Most shoulder pain does not come from one bad lift. It comes from hundreds of ordinary movements. Arms forward at a keyboard, hands on a wheel, a bag carried on the same side every single day. None of it feels like much at the time.

The ache shows up later, often on a quiet day, because that is when you finally notice it. Nothing new happened. The total simply caught up.

Shoulders are also slow to complain. The tissues around the joint tolerate a lot before they become sore, so by the time you feel an ache the pattern behind it has usually been in place for weeks or months.

What goes wrong inside the joint?

Your shoulder is a ball and socket joint with a very shallow socket. That is what gives the arm its huge range of motion. The trade-off is that the joint depends on a group of small stabilizing muscles, and on the shoulder blade behind it, to keep the ball centered as the arm moves.

When posture holds the joint in one position for hours, some of those muscles stay switched on while others stop contributing. The shoulder keeps moving, just not the way it was built to. Tissues that should glide begin to rub and pinch, especially when you reach overhead or behind you.

Why do my neck and upper back get involved?

The shoulder does not work alone. Reaching overhead needs the upper back to straighten and the shoulder blade to rotate. If the upper back is stiff and rounded from sitting, the shoulder has to make up the difference.

It runs the other way too. When the shoulder stops gliding properly, the muscles between the neck and shoulder work harder to lift the arm. That compensation is what turns a minor ache into neck tension, headaches, or pain that limits reaching, lifting and sleeping.

When should a sore shoulder be examined?

Have it looked at if any of these apply:

  • Pain that wakes you when you lie on that side
  • Trouble reaching overhead, behind your back or into a back pocket
  • A catching or pinching feeling at a certain point in the movement
  • The arm feels weak when you lift or carry
  • The ache has lasted more than two weeks or keeps returning

Will it go away on its own?

A mild ache from a busy week often settles when the pattern changes. An ache that keeps returning usually means the mechanics have not changed, and waiting tends to let the shoulder stiffen further. The earlier the movement is restored, the less there is to undo.

What helps day to day?

The aim is to break up the positions that hold the shoulder still and bring back the movements it has been missing.

  • Rest your forearms on the desk or armrests so your shoulders are not holding your arms up
  • Keep the mouse and keyboard close so you are not reaching forward for hours
  • Switch the side you carry a bag on, or use both straps
  • Every 30 minutes, roll your shoulders back and squeeze your shoulder blades together gently
  • Raise your arms overhead a few times a day within a comfortable range
  • Avoid sleeping with the arm pinned under you or stretched overhead

How does a chiropractor approach shoulder pain?

The first visit is a conversation and an exam. Your chiropractor checks how the shoulder, shoulder blade, neck and upper back move together, tests the strength of the muscles around the joint, and looks for the positions that reproduce your pain. Imaging is ordered only when the findings call for it.

If care is appropriate, it is aimed at restoring the mechanics so the joint no longer has to protect itself. That can include chiropractic adjustments to the upper back, neck and shoulder, hands-on work for tight muscles, and specific exercises for the stabilizing muscles. If the exam suggests a tear or another problem that needs a medical specialist, you are told that and referred.

When to seek urgent care

Call 911 for shoulder or arm pain that comes with chest pressure, shortness of breath, sweating, nausea or jaw pain, since these can be signs of a heart problem. Get urgent care for a shoulder injury after a fall with visible deformity, severe swelling, or an arm you cannot lift.

Common questions

Could my shoulder pain be coming from my neck?

Yes. Irritated nerves in the neck can refer pain to the shoulder and arm. An exam of both areas shows which is driving it.

Should I rest it completely?

Usually not. Keeping the shoulder still for long periods tends to make it stiffer. Gentle movement within a comfortable range is better unless you have been told otherwise.

Is it a rotator cuff problem?

It can be, since those are the stabilizing muscles described above. Only an exam, and sometimes imaging, can tell you.
