- Check in at the front desk : When you walk in, our friendly receptionist will greet you and get you started with your new patient paperwork.
- Complete your intake forms : The forms are brief and straightforward. They cover your basic personal information, health history, and current concerns. Our front desk team is right there to answer any questions and help you through each section.
- Insurance verification : In most cases, our team will have already verified your insurance and benefits before you even walk through the door. When you call to book your appointment, we collect your insurance details so we can confirm your copay and the number of covered visits ahead of time. That way, there are no surprises when you arrive.
- Meet with the doctor : Once your paperwork is complete, your doctor will sit down with you for an in-depth consultation to understand your pain, your lifestyle, your work habits, and your wellness goals, so your care plan is truly built around you.
How Long Does the Paperwork Take?
The intake forms are designed to be quick and easy. Most patients finish in just a few minutes. We keep them focused on the essentials so that your doctor has enough information to begin your evaluation without taking up too much of your time. We recommend arriving about 10 to 15 minutes before your scheduled appointment to allow time to complete your forms comfortably.
